What we do for disqus in the DocBook is concatenate the book's xml:id
and the section's xml:id. This requires that your book xml:ids be unique
within a Disqus "Forum", but I believe Anne sets up a separate "Forum"
for each book. At Rackspace we have one big forum, so we have to take
care that our book xml:ids are unique.
