[Openstack-docs] Title of the main page appears twice in the Web Browser Title

Yeah, the <head><title> is a concatenation of the topic title and the
book title. On the index.html page that happens to be the same title. It
would be easy enough not to write out the title twice in that case though.

Ok, I have a fix and will push through a git review for the plugin soon
(something's not right with the setup at the moment...will ask the infra
folks about that).
