The Ops session on the taxonomy of failure can use some input even before the session itself! There is also a google spreadsheet here: https://docs.google.com/spreadsheets/d/1sekKLp7C8lsTh-niPHNa2QLk5kzEC_2w_UsG6ifC-Pw/edit?usp=sharing that has a couple of examples already in it that would be a great way to collect data. Please go out to the etherpad, read and think about the issue if you are interested, and comment ahead if possible. It will provide more material for the discussion. It also is a way to provide your input even if you can't be at the summit.
