I totally support this effort, but I'm wondering if saying "The OpenStack UX project, with support of the OpenStack Foundation and the Technical Committee, is distributing..." wouldn't be more accurate and give credit where credit is due ?

Current wording make it look like the TC is the author of the survey.

Agreed, and I too support the effort.

I don't think attaching the TC to it gives you any better return on the survey. So with the goal of increasing numbers, our individual networks may help you, but I don't need the TC name attached to send and promote the link.
