The free version of survey monkey doesn't allow for question logic or sending to a large list of people at once (if we wanted to use some kind of email list). The reporting isn't so great either.
I'm sure someone should be able to expense the $40 or so to get a subscription for a few months. I'm willing to do it if you'd like.

Sorry for my slow reply, but it looks like we're making progress on the screener / questionnaire -- thanks to all those who've helped to move this along.
As to surveys, I saw mention of survey monkey -- am wondering if we're thinking the free or paid one:
SurveyMonkey offers a free basic version, useful for small and informal surveys and a paid version. The free version allows for very little customization of themes, a limited number of templates, a few already created surveys, no downloads of reports or data, and 15 different question types. You can include up to 10 questions and collect up to 100 responses per free survey.
SurveyGizmo mentioned also has a free and paid version.
Another option is GoogleForms. Google Forms, which is free, has been an easy and accessible way to collect large amounts of data.
Depending on budgets or getting funding, maybe we should consider Google Forms?
