It looks like you actually had three different accounts, which
may have either been imported through OpenID from separate Launchpad
accounts or the result of an account merge in Launchpad. Only your
oldest two accounts (created in December and February)
submitted contact information and uploaded an SSH key, though all
three (including the newest account from March) agreed to the
ICLA. Further, the oldest of them was the only one with an SSH
username.

I've done what I can to try to clean it up, and hopefully the
account you're authenticating to via SSH and the WebUI will be the
same going forward and will be selectable for reviews and group
memberships. Please let me know if this is not the case.
