[Openstack-docs] <th>, <td> inside table's <thead>

My patch

raised quite some discussion and confusion (it confused me as well).
Diane, David and myself discussed a bit further and suggest to use
<th> by default inside <thead> since that's the usual practice and the
way that HTML defines it (docbook says th is for row header only).

David will take care eventually that <td> in <thead> will be rendered
the same way as <th> but this is not a high prio job.

I'm now abandoning my patch to do a syntax check for this and will
update the conventions page,
