[cinder] db sync error upgrading from pike to queens

Brandon,

I am thinking you are hitting this bug: 
https://bugs.launchpad.net/cinder/+bug/1806156

I think you can work around it by retrying the migration with the volume 
service running.  You may, however, want to check with Iain MacDonnell 
as he has been looking at this for a while.
