I'm going to get this started later today using the "series of email
threads" mentioned above. I'll give them '[placement]' and '[ptg]'
and base them off whatever seems relavant from the two etherpads
[2,3]. As before, feel free to continue adding to the etherpads.

Also, note that I've framed in a retrospective etherpad [4] because
that's often useful. I've put some guidance on there, but to repeat
some of it:

* try to be blameless (in the sense of a devopsy blameless
   post-mortem)
* you don't need to restrict yourself to Stein
* avoid, at least for now, entering solution space: let's look retro
   first
