The ISV-WG reviewed the draft Hackathon application form today at goo.gl/08grC6, and has this feedback:

Modify "How many participants do you expect?", to "How many participants or teams of n people can you support (with space, equipment, etc.)?".
Add "Do you have the infrastructure to support the Hackathon or will you need assistance?"
Add "What SDKs/tools do you plan to make available to participants?"
Add a link to a planning checklist to "As an organization, do you have the necessary installations, equipment and base knowledge to run an OpenStack Hackathon?", so they have a better idea of what will be required.
Thanks for the opportunity to review and provide feedback!
