Gerrit account "Open-E test environment" is disabled. This account was posting "Merge Failed." messages on each patchset incorrectly. Please also add "CI" to your account name as per the instructions here: http://docs.openstack.org/infra/system-config/third_party.html#creating-a-service-account .
