I've put a page on the wiki quickly to reflect the current questionnaire / screener at https://wiki.openstack.org/wiki/PersonaSurvey. It still needs some wordsmithing / formatting to fix it (hopefully someone might volunteer to fix it) or I'll get to it sometime soon.
